The Transitional Care Center (TCC) at Simi Valley Hospital operates as a transitional unit or a “stepping stone” in post-acute care. TCC will help you when your physician determines that you no longer require acute hospitalization, but you still need skilled nursing and rehabilitative care before you can safely return home.
A leader in quality care, the TCC is a licensed 44 bed distinct part skilled nursing facility specializing in providing sub-acute level skilled nursing and rehabilitation services. Our convenient hospital-based location in Simi Valley affords you the care you deserve.
While most patients’ average length-of-stay is less than 20 days, the actual time in our unit can vary. Each patient’s length-of-stay is primarily determined by their diagnosis, overall condition and progress. On a daily basis, most of our patients receive rehabilitation services including physical therapy, occupational therapy, recreational therapy and/or speech therapy services from our highly competent and skilled therapy staff. Since our TCC is located adjacent to our Acute Rehabilitation unit, our patients benefit from the outstanding skills of our rehabilitation team.
